THE EFFECT OF rhBMP-2 ON ALLOGRAFT UNION AND REMODELING IN A CORPECTOMY MODEL
نویسنده
چکیده
Introduction: Anterior corpectomy with instrumentation and fusion is common in the treatment of fractures, tumors, infections, and kyphotic deformities of the thoracic and lumbar spine. Both autogenous (iliac crest, rib or fibula) as well as allogenous strut grafts are utilized in the reconstruction. One of the advantages of autogenous bone is thought to be the endogenous cells and growth factors present within the graft to promote union. Recombinant human BMP-2 (rhBMP-2) in a collagen matrix has been evaluated as a means of increasing the likelihood of arthrodesis in an interbody (single motion segment) allograft reconstruction. However, we are unaware of similar studies of cortical strut allograft healing after vertebral corpectomy. The purpose of this study was to evaluate healing and incorporation of strut allografts after lumbar corpectomy, and to evaluate the use of rhBMP-2 to augment graft union and remodeling.

Knockdown of toll-like receptor 4 signaling pathways ameliorate bone graft rejection in a mouse model of allograft transplantation
Non-union occurring in structural bone grafting is a major problem in allograft transplantation because of impaired interaction between the host and graft tissue. Activated toll-like receptor (TLR) induces inflammatory cytokines and chemokines and triggers cell-mediated immune responses. The TLR-mediated signal pathway is important for mediating allograft rejection. We evaluated the effects of ...
متن کاملComparison of Auto, Iso, and Allografts and the Effect of Nsaids Onbone Healing in a Novel Murine Segmental Femoral Model
Introduction: Structural bone allografts are frequently used to reconstruct bone defects in the extremities as well as for spinal fusion surgery. Failed healing at the graft-host junction is one of the most frequent complications of allograft surgery. However, the factors that regulate healing and graft incorporation remain poorly understood. The goal of these studies is to examine the molecula...
متن کاملThe effect of bone allografts combined with bone marrow stromal cells on the healing of segmental bone defects in a sheep model
BACKGROUND The repair of large bone defects is a major orthopedic challenge because autologous bone grafts are not available in large amounts and because harvesting is often associated with donor-site morbidity. Considering that bone marrow stromal cells (BMSC) are responsible for the maintenance of bone turnover throughout life, we investigated bone repair at a site of a critically sized segme...
متن کاملFreeze-Dried Platelet-Rich Plasma Accelerates Bone Union with Adequate Rigidity in Posterolateral Lumbar Fusion Surgery Model in Rats
Fresh platelet-rich plasma (PRP) accelerates bone union in rat model. However, fresh PRP has a short half-life. We suggested freeze-dried PRP (FD-PRP) prepared in advance and investigated its efficacy in vivo. Spinal posterolateral fusion was performed on 8-week-old male Sprague-Dawley rats divided into six groups based on the graft materials (n = 10 per group): sham control, artificial bone (A...
متن کاملMicroarray Analysis of Gene Expression Reveals that Cyclo-oxygenase-2 Gene Therapy Up-regulates Hematopoiesis and Down-regulates Inflammation During Endochondral Bone Fracture Healing
BACKGROUND Cyclo-oxygenase-2 (Cox-2) is an inflammatory mediator that is necessary for the tissue repair, including bone fracture healing. Although the application of Cox-2 gene therapy to a murine closed femoral fracture has accelerated bony union, but the beneficial effect was not observed until the endochondral stage of bone repair that is well after the inflammatory stage normally subsides....
